Thomas nearly dies many times in the series, and there are several instances in which he knowingly puts his life at risk. However, he is still very much alive.
Some of his friends and closest allies and even love interests sacrifice themselves so that he can live (such is the case with Teresa), but Thomas never forgets them and tries not to blame himself for their deaths. I believe that once people realized that Thomas himself is basically the cure, his life became much more challenging, and he wis almost constantly in danger. I also think that losing his closest friends deeply hurts him and perhaps even kills his fiery will and unbeatable spirit, as emotional pain and suffering can sometimes be stronger than any physical pain.
