Identify and explain/justify the dietary advice that you would give to the following patients who attend a dietetics clinic: a. Eman is a female patient aged 25 years with Type 2 diabetes mellitus. Her BMI is 32 kg/m2. b. Richard has a BMI of 36kg/m2, has recently had a heart attack, and has been diagnosed with coronary heart disease. Identify the changes to the diet that need to be made and, for each change that you recommend, explain the reasons for the change.

This question is technically asking for two separate answers that address the possible dietary changes for the two listed patients. The following answer will focus on patient #2: Richard.
To start, it is tough to give advice on possible dietary changes without knowing what some of Richard's current diet contains. Based on his BMI being a 36, I would assume that his current diet is likely calorie intensive and contains far more fatty foods than recommended. A BMI of 36 puts Richard in the obese category. His recent diagnosis of coronary heart disease supports the likelihood of a high fat, high cholesterol diet. Coronary heart disease occurs when the major blood vessels that supply the heart are damaged. The usual culprit is cholesterol containing deposits (plaque) inside the arteries. The plaque buildup narrows the blood vessels and reduces blood flow which forces the heart to work harder.
The question indicates that it is asking for dietary advice. Richard needs to do a fair number of things. One piece of advice to give him is about portion control. This will help him reduce his overall caloric intake. A portion of meat (protein) is 3 ounces. This is equivalent in size to a deck of cards. He should only be consuming about 2,000 calories per day.
Next, Richard needs to focus on what he is eating. Dietary guidelines recommend that up to 65% of a person's diet should come from carbohydrates. 35% of the diet should be protein, and no more than 20% should come from fats. Richard should focus on consuming unsaturated fats instead of trans-fats and saturated fats. Proteins should be lean proteins (like fish and chicken); he needs to avoid red meat. Finally, Richard should focus on consuming fruits and vegetables for his "snack" foods.
